If you are exempt from paying for your NHS treatment you will need to tick the correct box on the back of the NHS form. Please ensure you know which exemption is correct for you as the practice cannot take resposibility for any form incorrectly filled out and the NHS is likely to fine you. We will not ask to see proof of exemption as it is not the  responsibility of the practice to check, it is the responsibility of the patient filling in the form. However it may be useful for you to bring in your exemption details to ensure you fill in the form correctly. We are also not allowed to help you fill in the form or sign it on your behalf.
