We are currently accepting new patients. The easiest way to register is to call the reception during opening hours. You will be asked for a few details which will be used should the need to contact you about your appointment arise.
On your first visit it's best to turn up slightly early as there is a short medical form to complete. It's important that this is accurate as your medical history may have an impact upon the way certain treatments are performed. The medical form also includes your doctor's details and your NHS number. It's important we have these in case you ever need to be referred for any treatment. Your information will not be shared with any third parties unless it is necessary for a referral to another service which requires your verbal consent.
At the start of every course of treatment a NHS form has to be filled in by every patient.
Exempt patients
If you are exempt from paying for your NHS treatment you will need to tick the correct box on the back of the NHS form. Please ensure you know which exemption is correct for you as the practice cannot take responsibility for any form incorrectly filled out and the NHS is likely to fine you. We will not ask to see proof of exemption as it is not the responsibility of the practice to check, it is the responsibility of the patient filling in the form. However it may be useful for you to bring in your exemption details to ensure you fill in the form correctly. We are also not allowed to help you fill in the form or sign it on your behalf.
Your first visit
The first visit is usually an examination which often includes radiographs (x-rays)and a discussion about any treatment you may require. If there is no other treatment required and you need basic scaling this can often be done too. If treatment is needed this will be arranged for your next appointment and a treatment plan outlining cost and likely number of visits required will be given to you.
If you feel you need emergency treatment rather than an examination please let the receptionist know at the time of booking.
Dental insurance
If you are planning to claim back the cost of your treatment through your insurance please make the dentist aware as insurance companies usually request a different type of treatment plan form to be printed out.
Once you have been seen you will be a registered patient with us.
